Patient Rights and Responsibilities
A Patient's Bill of Rights
You have the right to receive quality care and treatment consistent with available resources and generally accepted medical standards in a safe environment. When applicable, WBAMC involves your family in care, treatment, and services decisions to the extent permitted by you or your surrogate decision-maker in accordance with law and regulation.
RESPECT AND DIGNITY
WBAMC respects your right as a patient to considerate and respectful care, with recognition of your personal dignity.
PRIVACY AND CONFIDENTIALITY:
WBAMC implements safeguards to ensure the confidentiality, integrity, and availability of your protected health information and similar personally identifiable information whether it be in electronic, written and/or verbal form.
You have the right not to be discriminated against based on age, race, ethnicity, religion, culture, language, physical or mental disability, socioeconomic status, sex, sexual orientation, and gender identity or expression.
You have the right to know the names of those who are taking care of you.
COMMUNICATION AND INTERPRETIVE SERVICES:
WBAMC will provide interpretative and translativeservices as well as vision, speech, hearing, or cognitive assistance devices in a manner that meets your specific needs.
You have the right to any and all necessary information, in understandable terms, to make knowledgeable decisions on consent or refusal for treatment, or participation in clinical trials or other research investigations as applicable. Such information is to include any and all complications, risks, benefits, ethical issues, and alternative treatments as may be available.
REFUSAL OF TREATMENT:
You have the right to refuse care, treatment and services to the extent permitted by law and existing government regulations and to be informed of the medical and administrative consequences of your refusal for treatment.
TRANSFER AND CONTINUITY OF CARE:
When medically permissible, you may be transferred to another hospital only after receiving complete information and an explanation concerning the needs for and alternatives to such a transfer.
WBAMC accommodates your right to religious and other spiritual services.
You have the right to appropriate assessment and management of your pain, information about pain relief measures, and to participate in pain management decisions.
You have the right to request a chaperone be present when being examined or if you feel uncomfortable during routine appointments.
WBAMC allows a family member, friend, or other individual to be present for emotional support during the course of your stay. Additionally, WBAMC must inform you (or your support person) of your visitation rights, including any clinical restrictions or limitations.
If you are unable to make decisions about your care, treatment, and/or services, WBAMC respects your designated surrogate decision-maker in making these decisions.
EXPLANATION OF CARE:
You have the right to an explanation concerning your diagnosis, treatment, procedures and prognosis of illness in terms that are easily understood.

You are responsible for providing accurate and complete information about your health to all those involved in your care.
ADHERENCE WITH MEDICAL CARE:
You are responsible for adhering to the medical and nursing treatment plan, including follow-up care, recommended by healthcare providers.
RESPECT AND CONSIDERATION:
You are responsible for being considerate of the rights of other patients and hospital personnel.
WBAMC files and maintains your records for you in a safe and confidential manner. If you require a copy of your record, you are responsible for making such a request through WBAMC's Release of Information/Correspondence Office.
HOSPITAL RULES AND REGULATIONS:
You are responsible for following WBAMC rules and regulations that relate to visitor and patient conduct.
You are responsible for informing your health care provider whether or not you understand your treatment plan and what is expected of you. You are responsible to ask questions or bring forward doubts at the time of treatment.
You and your visitors have the responsibility to report any unsafe situation that involves either health care treatment or the hospital environment. Report any such situations to any staff member or Supervisor. For any questions or concerns, the Patient Safety office at 915-742-3696.
