The Rio Bravo Medical Home is an Army-run, primary care clinic, located off-post on the East side of El Paso. Our intent is to bring high quality army healthcare closer to where our families live.

In most healthcare systems, they don't engage the patient until the patient reaches out for care, when you enroll to Rio Bravo Medical Home, you will be assigned a team of highly qualified medical professionals who will provide you with a personalized, comprehensive care plan that meets your individual needs. We want you to take an active role in your own healthcare. Your healthcare team will work with you to provide preventive screening and services, manage chronic health concerns and take care of new health issues as they arise.

Your team consists of nurses, doctors, nurse practitioners, pharmacy staff, behavioral health staff and others who will collaborate with you in your care. However, in the Rio Bravo Medical Home, you are an active partner in coordination, communication, and decision-making— you are the center of your own personal healthcare team.

If you receive care outside the Medical Home, such as emergency room care, inpatient hospital care or a subspecialist’s care, our team will coordinate that care and ensure it is integrated into your personal comprehensive care plan.

  • Make sure you know and understand all of your options for care
  • Help you decide what care is best for you. Sometimes more care is not better care.
  • Ask you for feedback about your experience getting care.

3. Support You in Caring for yourself

  • Make sure you leave the office with a clear idea of how to care for yourself
  • Help you set goals for your care, and help you meet your goals, one step at a time
  • Give you information about classes, support groups, or other types of services to help you learn more about your condition and stay healthy.

Army Community Based Medical Homes increase access to primary care, expand the definition of healthcare quality and reinvigorate our commitment to customer service.

QUALITY

In most healthcare systems, they don’t engage their patients until the patient reaches out for care. In the Community Based Medical Home, the healthcare team develops a comprehensive care plan as soon as the patient enrolls and then proactively engages the patient as a partner in care.

Each day begins with a “Huddle”, where the day’s care plans are reviewed and discussed prior to meeting with you. Contributions to the huddle come from a multidisciplinary team including the patient’s primary care manager, team nurses, nurse case managers, behavioral health professionals, clinical pharmacists and others.

When a patient receives care outside of the Community Based Medical Home, in the emergency room, hospital, or sub- specialist’s office, the patient’s care team ensures that care is coordinated and integrated into the comprehensive care plan. This focus on comprehensive, proactive care, team communication, and coordination of care advances our commitment to patient safety and healthcare quality.

The Community Based Medical Home is committed to a higher level of customer service. That higher level of customer service starts with same day service, giving you access to care when you need it.

From the way you are greeted when you enter or contact the clinic, to patient education, to your interaction with the healthcare team, we strive to build your trust in Army Medicine and make it easy for you to say that Army Healthcare is the best available.

ACCESS

In a traditional healthcare setting, the only way to gain access to care is through a face-to-face visit between patient and healthcare provider. We continue to believe that this encounter is the heart of the Community Based Medical Home experience. The face-to-face encounter builds trust, improves communication, and lays the groundwork for providing continuous care by your personal healthcare team.

The Community Based Medical Home starts with the face-to-face encounter and expands from there. Both patients and the healthcare team can take advantage of telephone and web-based communication to follow-up on health issues and concerns, share and update information, and coordinate care delivery.

In addition, we offer the opportunity for group visits, a coordinated interaction between the healthcare team and patients sharing the same condition. Furthermore, you will have the option to interact with members of the extended care team such as pharmacists, behavioral health professionals, nurses, and others. These expanded access options give patients more flexibility to manage their healthcare while ensuring access to the care they need.

Emergency

If you have an Emergency, please proceed to the nearest Emergency Room. An Emergency is defined as a condition that poses risk to life, limb or sight.

Non-Emergent patients can expect a long wait, referral to quick care or referral back to the Primary Care Manager. Rio Bravo Medical Home offers same dame appointments. If you do not have an Emergency, it is best that you call the appointment line and book an appointment with your PCM.

All Rio Bravo appointment requests for same day appointments will be honored as long as they call by noon. Calls to the appointment line after 12 PM will be accommodated as soon as possible same day or next morning. (This applies to Rio Bravo enrolled patients only)

The Rio Bravo Medical Home staff is available from 7:30 AM - 4:30 PM Monday - Friday. We are closed on weekends, all federal holidays and some associated training holidays.

The clinic will also close from 12:30 PM – 4:30 PM on the 2nd and 4th Thursdays of the month for staff training.

Many of the visits to a primary care practice and over 80% of all Emergency Room visits are for minor health issues that can be managed at home, with the right resources.

Self-Care is about becoming an active participant in your own healthcare, recognizing and taking care of small health concerns before they become too bad to self-manage.

The following resources are to assist you in learning what you can do to take care of yourself and when you DO need to seek care from a professional.

Pharmacy Self Care and Over the Counter Program

Did you know that many of the medications you may be seeking with a primary care visit are available to you over the counter at the pharmacy counter, free of charge?

The following medications can be obtained at any William Beaumont Pharmacy, free of charge, with a self-care pharmacy card:

APPOINTMENT CANCELLATIONS

Appointment Cancellations: There are three quick options to cancel appointments:

  1. For registered TRICARE Online (TOL) users, go to https://www.tricareonline.com , log-in and select CANCEL APPOINTMENT in the appointment center.
  2. Call the WBAMC Central Appointments at (915)742-2273 and follow the prompts.
  3. Select CANCEL during an appointment reminder phone call when prompted during the call.

The solution can be as simple as a phone call. Cancelling an appointment at least 24 hours in advance gives somebody else the chance to see their health care team.

Trusted Self Care Management Resources:

Relay Health is a secure message management service that is used to support Army PCMH. One of the tenets of PCMH is to provide the patient an alternate way to communicate with the Medical Home to avoid unnecessary trips to the clinic for administrative issues or to obtain information. Relay Health has a secure server that allows confidential information to be exchanged between a patient and their PCM without fear of interception by unintended recipients

Relay Health offers much more than just a way to message your Medical Home. It is also a way to request appointments and medication renewals, ask questions of either the Medical Home Nurse or PCM, and to receive reminders of future appointments or scheduled testing. Enrollees have access to information specific to their interest or condition—there are approximately 5000 patient information handouts available to search at leisure. It is also a way for the Medical Home to send important information to the patient population (such as when flu immunizations are available or to announce dates when the clinic is closed).

To Enroll in RelayHealth, ask the front desk or screening nurse when you come in for a visit, or go to www.RelayHealth.com from your home computer and ask to enroll. In order to enroll you will need an e-mail address, the name of your PCM and Medical Home. After you are enrolled, the team at Relay Health will contact you to obtain permission from you to activate your account. After that you can use the service as often as you’d like and there is no cost to you. Ask the clinic staff if you have questions.

What to bring:

-Current list of your medications OR all your prescription bottles
-For your safety and to improve the quality of care you receive we strongly recommend that you carry a list of your medications with you to all of your medical visits. If you do not have an accurate list, you can create one using this printable worksheet (or another form that you prefer). Additionally, during your visit at the Internal Medicine Clinic we can provide you with a current medication list.
-Print and fill out a copy of our check-in worksheet

-If you have been seen by our clinic after April 1 2013, please print this follow up worksheet

-If you have not been seen by our clinic since April 1 2013, please print this new worksheet

-Medical records from non-military treatment facilities
For example, network specialists, non-WBAMC ER visits, Colonoscopy reports
-Vaccination records
For all vaccinations completed outside a military treatment facility.

-Please arrive 15 minutes before your appointment time to allow adequate time for parking, check-in and screening. If you are checked-in more than 10 minutes late, we will do our best to work you into the schedule later that day OR you will have the option to reschedule your appointment.
